Personalized Care
Successful aging in place and maintaining independence in a residential environment starts with identifying any potential therapy needs. At The Blake, new residents moving in can coordinate with the senior living staff and medical team to begin a screening process to assess current risks. This can include:
- Fall risks
- Safety in home environment
- Safety in community areas
- Musculoskeletal deficits
- Neurological declines
- Cognitive declines
Our therapy team also assists with equipment recommendations and providing home adaptive strategies to ensure each resident’s new home is safe, comfortable and best meets their needs. They’ll continue to engage with residents to address any new medical concerns, collaborating with primary care physicians and working with the resident in their home environment to reach personal and health-related goals.
